Waiver Form

PLEASE READ CAREFULLY. The waiver form is completed and signed online when you sign up. It collects:

• Parent/Guardian's full name, email and phone
• Each child's name and age
• Medical information — please inform us of any medical conditions that may arise while attending BigStar Circus classes
• If applicable, the name of the before or after school or school holiday program you are from

WAIVER AND RELEASE OF LIABILITY FORM

I hereby, for myself, my heirs, executors, administrators, assigns, or personal representatives (hereinafter collectively, “Releasor,” “I” or “me”, which terms shall also include Releasor’s parents or guardian if Releasor is under 18 years of age), knowingly and voluntarily enter into this WAIVER AND RELEASE OF LIABILITY and hereby waive any and all rights, claims or causes of action of any kind arising out of my participation in the Activity.

I, acknowledge, that I am voluntarily participating in physical exercise that can be strenuous and subject to risk of serious injury during PRIVATE OR GROUP TRAINING.

BigStar Circus urges you to obtain a physical examination from a doctor before beginning any exercise or training program. I agree that by participating in these physical exercise sessions or personal training activities, I do so at my own risk. I recognize that exercise is not without some risk to the musculoskeletal system (e.g. sprain, strain) and cardiorespiratory system (e.g. dizziness, fainting, abnormal heartbeat, discomfort in breathing, abnormal blood pressure response, and in rare instances, heart attack or stroke). By signing this waiver, I acknowledge that not all risks can be known in advance.

I hereby release and forever discharge BigStar Circus, their affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns (collectively “Releasees”), from any physical or psychological injury that I may suffer as a direct result of my participation in the aforementioned Activity.

I further agree to indemnify, defend and hold harmless the Releasees against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, including attorney fees and any related costs.

I further acknowledge that Releasees are not responsible for errors, omissions, acts or failures to act of any party or entity conducting a specific event or activity on behalf of Releasees.

Media Release Consent

I give permission for BigStar Circus to take photographs and/or video footage of my child and/or myself during classes or events.

Emergency Medical

In the event that I should require medical care or treatment, I authorize BigStar Circus to provide all emergency medical care deemed necessary, including but not limited to, first aid, CPR, the use of AEDs, emergency medical transport, and sharing of medical information with medical personnel.

I further agree to assume all costs involved and agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. In the event that any damage to equipment, field or facilities occurs as a result of my or my family’s or my agent’s wilful actions, neglect or recklessness, I acknowledge and agree to be held liable for any and all costs associated with any such actions of neglect or recklessness.

BigStar Circus Free Trials — Terms & Conditions

By participating in the BigStar Circus Free Trial offer, you agree to the following terms and conditions:

🎪 Eligibility

• Offer is valid for new students only who have not previously attended regular term classes at BigStar Circus.
• Children must be within the suitable age range for each class type and must be able to safely participate in circus activities.
• A parent or guardian must complete the online waiver form before the child attends their first trial class.

🆓 Free Trial Details

Each child is entitled to one week of free classes, which includes up to three trial classes, each in a different skill set:

• Circus Acrobatics
• Circus Fusion
• Aerial Skills

• All trial classes must be booked in advance and taken within a 7-day period.
• Class times and availability are subject to current scheduling and capacity limits.

⚠️ Conditions

• Offer is non-transferable and may only be redeemed once per child.
• Missed trial classes cannot be rescheduled or carried over beyond the 7-day trial period.
• BigStar Circus reserves the right to refuse or cancel a trial booking if the child is not suited for the class level or if safety requirements are not met.
• Participation in the trial does not guarantee a permanent spot in a class. Enrolment must be completed separately to continue after the trial week.

📝 Additional Notes

• Trial classes are group-based and structured to give children a broad overview of BigStar Circus programs.
• Students should wear comfortable activewear and bring a water bottle.
• BigStar Circus reserves the right to modify or end this offer at any time without notice.

BigStar One Week Free & Merch Pack Promotion — Terms & Conditions

By participating in the BigStar Circus promotion, you agree to the following terms and conditions:

🆓 Free Trial Week

Offer includes one free week of classes, consisting of three trial classes across three different skill sets:

• Circus Acrobatics
• Circus Fusion
• Aerial Skills

• All three trial classes must be booked and attended within 7 consecutive days.
• Offer valid for new students only (i.e. children who have not previously attended term classes at BigStar Circus).
• Bookings are essential and subject to class availability.
• Waiver form must be completed by a parent or guardian prior to participation.

🎁 BigStar Merch Pack

Each new student who signs up under this promotion will receive a BigStar Merch Pack including:

• Items selected by BigStar Circus staff
• Entry into the Star Rewards Program
• A Skill Booklet linked to the Star Rewards Program
• Two promotional vouchers:

🎟️ 1. Bring a Friend Free Pass

• Allows the new student to bring one friend to a trial class free of charge.
• The class must be prearranged with BigStar Circus staff.
• The friend’s parent/guardian must complete the waiver (available on the BigStar Circus website) before the class.
• Subject to space and class availability.

🎫 2. Free Holiday Workshop Voucher

• Valid for one holiday workshop.
• Becomes active after the new student has signed up for a subscription and attended 8 classes during the current or most recent term.
• The voucher is non-transferable and subject to availability in the selected workshop.

⚠️ General Conditions

• This promotion may only be redeemed once per child.
• Offers are not redeemable for cash or credit.
• BigStar Circus reserves the right to change or withdraw the offer at any time without notice.
• Participation in this promotion implies acceptance of these terms and conditions.
